I hereby apply for membership of The Sri Lanka Institute of Directors and agree to be bound by its rules and by-laws, from time to time in force. I confirm that all information presented in this Application is true and accurate and that I shall cease to be a member of the Institute in the event that any such information is found to be false. I confirm that I do not have any unspent criminal convictions, that I am not an undischarged bankrupt, that I am not disqualified by court order from being a Director/Senior Manager/Sole Proprietor/Partner and that I know of no reason whatsoever which would disqualify me from holding such post/s. I undertake to conduct myself in a manner, which would be expected of a member of a reputable professional institute. I further undertake not to represent publicly the views of the Institute or to claim its support without the prior written approval of the Management of the Institute and I have read and agree to the Terms and Conditions and Privacy Policy
